CVS Health is seeking a dedicated and experienced Remote Case Manager Nurse to join our dynamic team. As a Case Manager Nurse, you will play a crucial role in the coordination and case management of patient care. You will work closely with patients, their families, and healthcare providers to ensure optimal health outcomes by managing care plans and providing ongoing support.

Requirements

• Active Registered Nurse (RN) license in good standing


• Minimum of 3 years of clinical experience, preferably in case management, care coordination, or a related field


• Strong knowledge of healthcare systems, insurance plans, and patient care processes


• Excellent communication and interpersonal skills


• Ability to work independently and manage time effectively


• Proficiency in using electronic health records (EHR) and case management software


• Reliable internet connection and a dedicated workspace free from distractions

Responsibilities

• Assess and evaluate patient needs through telephonic or virtual consultations


• Develop and implement individualized care plans in collaboration with the patient, their family, and healthcare providers


• Monitor patient progress, identify any barriers to care, and adjust care plans as necessary


• Coordinate with healthcare providers, insurance companies, and other resources to facilitate patient care


• Provide education and support to patients and their families about health conditions, treatment options, and self-care strategies


• Ensure accurate documentation of patient interactions and care plans in the electronic health records system


• Participate in continuous professional development and stay current with industry best practices

Educational Qualifications

• Bachelor of Science in Nursing (BSN) preferred


• Current RN license in the state of residence


• Certification in Case Management (CCM) is a plus

Experience

• Minimum of 3 years of clinical experience in a healthcare setting


• Experience in case management, care coordination, or utilization review is highly desirable
