WesternU COP MTM Call Center Patient Advocate

The WesternU MTM call center patient advocate will be responsible for calling patients regarding medication adherence such as 90-day fills, no refills, PDC challenges, and concerns regarding newly started medications.
The patient advocate will be assigned patients from the MTM call center team.
Knowledge, Skills and Abilities
? Be a licensed pharmacist or a recent grad from the class of 2022 with an active intern license.
? Have a good clinical understanding of chronic diseases such as hyperlipidemia, diabetes, and hypertension
? Demonstrated oral and written communication skills
? Willingness to work and learn in a team environment

A licensed pharmacist or a recent grad from the class of 2022 with an active intern license.

Essential Job Duties
Key Responsibilities
? Call patients regarding medication adherence
? Uncover challenges related to medication adherence
? Provide patients with suggestions and resources on how to improve medication adherence
Key Responsibilities
? Contact pharmacies, physicians, and medical groups as needed to help improve overall care of the patient related to medication adherence.
Key Responsibilities
? Document call success rates and escalations within the provided excel template
? Document successful calls within Practice Fusion EHR
? Sign off on one?s own documented calls within Practice Fusion
Key Responsibilities
Attend training sessions and call center meetings.
