Job Description

CVS Health is seeking a dedicated and compassionate Virtual Health Support Nurse to join our team. As a Virtual Health Support Nurse, you will provide essential healthcare support to our patients through telehealth services, ensuring they receive high-quality care from the comfort of their homes. Requirements

1. Current and unrestricted RN license in the state of residence.

2. Minimum of 2 years of clinical nursing experience.

3. Experience with telehealth or virtual care platforms is preferred.

4. Strong communication and interpersonal skills.

5. Ability to work independently and manage time effectively.

6. Proficiency in using electronic health records (EHR) and other healthcare technology systems.

7. High-speed internet connection and a dedicated, quiet workspace.

Responsibilities

1. Provide virtual health consultations and patient assessments via telehealth platforms.

2. Develop and implement individualized care plans for patients with chronic conditions.

3. Educate patients on disease management, medication adherence, and lifestyle modifications.

4. Coordinate with physicians, pharmacists, and other healthcare team members to ensure comprehensive patient care.

5. Monitor patient progress and follow up on care plans to ensure optimal health outcomes.

6. Document patient interactions and care plans accurately in the EHR system.

7. Participate in ongoing training and professional development to stay current with best practices in virtual healthcare.

Educational Qualifications

– Bachelor? Degree in Nursing (BSN) preferred.

Experience

– Minimum of 2 years of clinical nursing experience.
