Job Description
As a Telehealth Nurse Navigator at CVS Health, you will be responsible for guiding patients through their healthcare journey. Your role will involve providing telephonic support, care coordination, and patient education to ensure that patients receive the appropriate care and resources they need. You will be a critical part of our patients’ healthcare team, working closely with physicians, pharmacists, and other healthcare professionals to enhance patient outcomes and satisfaction.
You will need to be well-versed in telehealth technologies and comfortable working in a remote environment. The ability to communicate effectively, show empathy, and maintain patient confidentiality is essential. Your primary goal will be to support patients in managing their health conditions, adhering to treatment plans, and navigating the healthcare system efficiently.
Requirements
– Active Registered Nurse (RN) license in the state of residence
– Minimum of 3 years of clinical nursing experience
– Experience in telehealth or case management preferred
– Strong communication and interpersonal skills
– Proficiency in using electronic health records (EHR) and telehealth platforms
– Ability to work independently and as part of a multidisciplinary team
– Strong organizational and time-management skills
Responsibilities
– Provide telephonic support and education to patients regarding their health conditions and treatment plans
– Coordinate care with healthcare providers, pharmacists, and other members of the healthcare team
– Assist patients in navigating the healthcare system and accessing necessary resources
– Monitor and follow up on patients’ progress, ensuring adherence to treatment plans
– Document patient interactions and care plans accurately in the EHR
– Participate in continuous quality improvement initiatives
– Stay updated with current best practices and guidelines in telehealth and nursing care

Educational Qualifications
– Bachelor of Science in Nursing (BSN) preferred
– Active Registered Nurse (RN) license
Experience
– Minimum of 3 years of clinical nursing experience
