Department: COE Patient Navigation Department's Website: Summary of Job Duties: The RN III Cancer Center Navigator (RN Case Manager) is responsible for promoting wellness and improved outcomes within a defined population of individuals with long-term conditions and complex health needs. The RN Case Manager supports their appropriate utilization of healthcare services by providing patients with the support, education, and assistance necessary to prevent or maintain their disease and wellness. The RN Case Manager works closely with the individual patients, their families, and providers of care to develop and implement individual health plans. Additionally, they provide support to the patient in self-management, enabling the patient's optimal functioning and improved collaborative coordination of care. The RN Case Manager's goal is to reduce waste and inefficiency within the concept of the patient-centered medical home. This position will cover the Southeast Arkansas Region. The counties covered in the Southeast Arkansas region include Fulton, Randolph, Clay, Izard, Sharp, Lawrence, Greene, Stone, Independence, Jackson, Craighead, Mississippi, Poinsett, Cleburne, White, Woodruff, Cross, Crittendon, Pulaski, Lonoke, and Prairie. Qualifications: Minimum Qualifications: Graduate of an accredited Nursing Program with an associate degree. Minimum of three (3) years RN experience in a clinical environment with current BLS certification. Valid Arkansas Registered Nursing License. Certification in Care Management (CCM), must be obtained within one (1) year of the hire date. Proficiency in Microsoft Office suite. Skilled in Care management and Experience as a participant in continuous quality improvement. Additional Information: Key Responsibilities: Nursing: Health Education, Research, and Care Coordination Triage, assessment, planning, implementation, and evaluation of nursing care and patient education either by telephone or within the assigned UAMS clinic using the care management model. Conducts care management, including care plans, assessing progress and barriers.Coordinates care and/or follow-up for patients who receive care in inpatient and outpatient facilities. Helps develop patient?s Plan of Care (POC) per clinic standards utilizing assessment data. Evaluates the progress of patients toward the attainment of desired outcomes.Verifies and evaluates care delivered to ensure care is consistent with standards and regulatory requirements. Coordinates patient education coordination in collaboration with physician staff. Identify eligible patients and facilitate access to appropriate clinical care and clinical trials. Works with other research staff to coordinate screening, consenting, enrollment, treatment/observation, and follow-up of subjects in cancer trials. Ensures treating physicians and subjects proceed through treatment phases on schedule without deviation. Facilitates effective communication among staff, clinical professionals, care coordinators, patients, PIs, research staff, and other support system positions. Serves as facilitator for patient navigation. Responsible for coordination of patient navigation as a point of contact for a team of patient navigators, patient navigators, physicians, and others to enhance patient services. Ensure referrals for trials or supportive care and support services are made in a timely way; facilitate scheduling and monitor patients to ensure follow-through. Track individual patient?s progress along the care continuum; identify potential obstacles and perform appropriate interventions. Leadership Acts as a resource/support to others in the performance of duties.
