Job Description


CVS Health is seeking an enthusiastic and dedicated Remote Patient Care Coordinator to join our team. As a Remote Patient Care Coordinator, you will play a vital role in ensuring our patients receive the highest level of care and support. This position will involve coordinating patient care remotely, liaising with healthcare providers, and assisting patients with their healthcare needs.


Requirements


1. High School Diploma or equivalent.


2. Proven experience in a healthcare or customer service role.


3. Excellent communication and interpersonal skills.


4. Proficient in using computer systems and software.


5. Ability to work independently and as part of a team.


6. Strong organizational skills and attention to detail.


7. Must have a reliable internet connection and a quiet workspace.


Responsibilities


1. Act as the primary point of contact for patients, providing support and answering their queries.


2. Coordinate patient appointments and follow-ups with healthcare providers.


3. Maintain accurate patient records and ensure confidentiality.


4. Assist patients in understanding their treatment plans and medication instructions.


5. Collaborate with other healthcare professionals to ensure seamless patient care.


6. Provide information on CVS Health services and encourage patient engagement.


7. Identify and escalate patient issues to the appropriate healthcare provider or department.

Educational Qualifications
