Description

As a Remote Care Coordinator Nurse at CVS Health, you will play a critical role in providing exceptional care to our patients. Requirements

• Valid RN license in the state of residence


• Minimum of 3 years of nursing experience, preferably in case management or care coordination


• Proficient in using electronic health records (EHR) systems


• Excellent communication and interpersonal skills


• Strong organizational and time-management abilities


• Ability to work independently and manage multiple tasks


• Reliable high-speed internet connection and a quiet, dedicated workspace

Responsibilities

• Develop and implement individualized care plans for patients


• Coordinate with healthcare providers to facilitate patient care


• Provide patient education and support regarding their health conditions and treatment plans


• Monitor patient progress and adjust care plans as needed


• Conduct regular follow-up calls with patients and their families


• Document all patient interactions and care plans accurately in the EHR system


• Participate in team meetings and contribute to continuous improvement initiatives

Educational Qualifications

• Bachelor? degree in Nursing (BSN) preferred


• Current, valid RN license in the state of residence

Experience

• At least 3 years of nursing experience


• Experience in case management or care coordination is preferred
