? This role is for an afternoon/evening shift position. This position includes shifts starting 12-4pm EST and ending between 8p-12am EST including an alternating weekend rotation.
? Changes in schedule can be re-evaluated with seniority/company growth/performance over time.

RESPONSIBILITIES: ? Recommends goal directed medication therapy to reduce symptom burden ? Answers consults from care givers for hospice medication management ? Serves as subject matter expert (SME) for managing hospice medications ? Provides hospice clinical staff with education and training ? Evaluates drug therapy and communicates/documents findings and recommendations ? Recommends medications, including initiation, continuation, discontinuation and altering therapy, based on established protocols ? Assures optimal, effective pharmaceutical care and collaborative practice for hospice patients ? Maintains a current knowledge of all professional and technical developments and requirements affecting the defined area of responsibility ? Prepares information for, and attends IDG (Interdisciplinary Group) meetings when requested ? Processes admission, transfer, and discharge information by adjusting patient data in computer ? Performs other related duties as assigned to support achievement of Pharmacy goals and objectives.

REQUIREMENTS: ? RPh required, PharmD preferred ? Full, current and unrestricted license to practice pharmacy in the state of operation ? Citizen of The United States of America ? Must pass a designated and/or random drug screening ? Must pass a background/security review ? Proficient in written and spoken English ? Experience with various web browsers (Google Chrome / Firefox), Google Docs, Google Sheets or similar software ? Proven ability to manage multiple projects at a time while paying strict attention to detail ? Excellent listening skills ? Excellent verbal and written communication skills (typing)

PREFERRED EXPERIENCE:
? Completion of a PGY-1 or PGY-2 Residency in Palliative Care or Pain Management or equivalent experience in Hospice/Palliative Care/Pain Management.
? Board certification in a relevant specialty certified by the Board of Pharmaceutical Specialties
? Multidisciplinary team leadership experience in a variety of settings
? Experience in collaborative development and launch of new pharmacy or patient care services, including development of policies, procedures, tools and metrics
? Hospice experience
