Medical Coordinator - Remote | WFH
Position Summary
Join our team as a Medical Coordinator, where you'll play a crucial role in overseeing the care of 200 patients within our chronic care management and remote patient monitoring program. In this position, you'll be the key link between our patients and healthcare providers, ensuring seamless communication and top-notch healthcare delivery. Key Responsibilities
Monitor patient data and alerts from remote monitoring devices, ensuring prompt follow-up with patients and healthcare providers.
Engage with patients to develop monthly care plans, provide education on managing chronic conditions, and address any questions or concerns.
Maintain accurate and up-to-date patient records, adhering to healthcare regulations and privacy standards.
Collaborate with healthcare providers to tailor care plans to meet each patient's unique needs.
Qualifications
Experience in chronic care management or remote patient monitoring is a plus.
Exceptional organizational and multitasking abilities.
Strong communication and interpersonal skills.
Familiarity with electronic health records (EHR) systems is preferred.
Detail-oriented with proficiency in Microsoft Word and Excel.
