Responsibilities
? Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
? Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
? Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
? Functions as a liaison between disciplines and facilities during the admission process.
? Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
? Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
? Performs on-call responsibilities and on-call services to patients/families as assigned.
? Performs other duties as assigned.

Qualifications
? Current RN license, specific to the state(s) you are assigned to work.
? One year of clinical RN experience.
? Hospice experience in homes strongly preferred
? EMR: Home Care Home Base preferred
? Current CPR certification.
