The Care Management Support Professional 2 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
? Educate and guide Interdisciplinary Teams (IDT) through behavioral health policy and process.
? Ensure compliance with the Department of Health Services on written guidelines and procedures on restrictive measures.
? Lead and collaborate with the Behavioral Health and Wellness Team to maintain department documents (policies, guidelines, processes) and resources.
? Collaborate, support and educate IDT proactively and during challenging situations for members and providers
? Act as a liaison and partner between internal and external stakeholders. Provide education and support to IDT regarding behavioral health and wellness planning, county crisis planning and police safety planning. Communicate and collaborate with external stakeholders to support development of local partnerships.
? Lead the transition process for Institutions of Mental Disease (IMD) and State Intensive Treatment Programs (ITP). Create annual reports, collect data, and monitor timelines for IMD and State ITP admissions to the Department of Health Services. Provide ongoing collaboration, support, and education with internal and external stakeholders regarding status of IMD/ITP admission, discharge status, and post discharge needs.
? Project management of behavioral health initiatives and resourcing.
? Develop behavioral health curriculum including the IDT Staff Handbook and other resources.
? Manage and facilitate the behavioral health training and onboarding of new hires.

The Care Management Support Professional 2 Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Required Qualifications

? Bachelor's degree
? 2 years experience with Mental Health and Behavioral Health and one or more of the Family Care target groups
? Comprehensive knowledge in behavioral health and strength-based approaches.
? More than 5 years of Microsoft Office technical experience with Word, Excel, Outlook
? Strong written and verbal communication skills.
? Strong advocacy skills.
? Decision making skills regarding own work assignments and priorities
? Must be passionate about contributing to an organization focused on continuously improving consumer experiences
? Ability to work independently and as part of a team.

Preferred Qualifications
? Master's Degree
? 5 years experience with Mental Health and Behavioral Health and one or more of the Family Care target groups
? Demonstrated experience with Microsoft Access

? Work Location: Wisconsin
? Travel: up to 25% throughout the State
? Typical Work Days/Hours: Monday ? Friday, 8:00 am ? 4:30 pm CST

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$57,700 - $79,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
