Job Description : JOB SUMMARY This job provides telephonic case management services to members with mental health/substance use and co-morbid chronic medical conditions offering integrated and comprehensive interventions. The incumbent uses clinical and motivational interviewing skills to assess behavioral health, physical health, and social determinants of health needs to establish member driven care plans and mutually agreeable short and long-term goals. They support members in developing self-management skills and adopting positive behavior changes; identify and address gaps in care; resolve barriers to evidence-based care; help to coordinate comprehensive healthcare services and navigate the healthcare system; and link with virtual and community-based resources. The overall goal is to help members navigate the health care system and develop condition self-management skills through behavior change. ESSENTIAL RESPONSIBILITIES Function as a primary integrated case manager for members with mental health/substance use and co-morbid chronic medical conditions to provide comprehensive case management interventions. Conduct telephonic clinical and psycho-social assessments to address the health and wellness needs of the Organization's members. Utilize standard case management approach to manage members? conditions (Motivational interviewing; Develop plans of care; Establish a plan for contact to review progress/assess needs; Communicate with treating provider(s); Identify community-based resources for long-term maintenance). Collaborate with case and condition management teams to provide co-management activities for members who require additional services. Participate in ongoing training and educational activities related to behavioral health and medical topics. Pro-actively incorporate lifestyle improvement and prevention opportunities into member interactions and coaching. Provide education on evidenced based treatment. Provide members with resources and tools to assist in health-related decision making. Assess for social determinants of health and link members with community resources to eliminate barriers. Collaborate with key community and clinical partners. Other duties as assigned or requested.
