Job Description

CVS Health is seeking a dedicated and compassionate Remote Nurse Health Coach to join our team. As a Nurse Health Coach, you will play a crucial role in empowering our members to achieve their health goals through personalized coaching and guidance. This position offers the flexibility to work from home, allowing you to provide exceptional care and support from the comfort of your own space.

In this role, you will be responsible for building relationships with members, assessing their health needs, creating individualized care plans, and providing ongoing support and education. You will work closely with a multidisciplinary team to ensure that our members receive the highest quality of care and support.

Key Responsibilities

– Conduct initial health assessments and develop personalized care plans for members.

– Provide ongoing health coaching and support through phone calls, video consultations, and digital communication.

– Educate members on various health topics, including chronic disease management, nutrition, exercise, and lifestyle modifications.

– Collaborate with other healthcare professionals to ensure comprehensive care for members.

– Monitor and track member progress, adjusting care plans as needed.

– Document all interactions and care plans in the electronic health record (EHR) system.

– Stay updated on the latest health and wellness trends and best practices.

Requirements

– Active Registered Nurse (RN) license in the state of residence.

– Minimum of 3 years of clinical nursing experience.

– Strong knowledge of chronic disease management and health coaching principles.

– Excellent communication and interpersonal skills.

– Ability to work independently and manage time effectively.

– Proficiency in using electronic health records (EHR) and other digital tools.

– Commitment to ongoing professional development and education.

Educational Qualifications

– Bachelor’s degree in Nursing (BSN) preferred.

– Certified Health Coach (CHC) or similar certification is a plus.

Experience

– At least 3 years of clinical nursing experience is required.

– Previous experience in health coaching or case management is highly desired.
