As a Telehealth Disease Management Nurse, you will be responsible for managing the health outcomes of patients with chronic diseases such as diabetes, hypertension, and COPD. Key Responsibilities:

• Conduct virtual assessments and monitor patients’ health status.

Requirements

Minimum Qualifications:

• Active RN license in the state of residence.
• Bachelor? degree in Nursing (BSN) preferred.
• Minimum of 2 years of clinical nursing experience.
• Experience in disease management or chronic care management is a plus.
• Proficiency in using telehealth platforms and technology.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Educational Qualifications

• Bachelor? degree in Nursing (BSN) preferred.
• Active RN license in the state of residence.
