Position: Care Coordinator I (Medical Assistant, Hybrid-remote potential)

Do you have what it takes to be a Care Coordinator:
? Completion of a Medical Assisting or LPN education program required
? CMA/RMA or LPN required
? Experience with quality reporting preferred

A day in the life of a HSO Care Coordinator:
? Provide telephone outreach and in-person consultation to members identified as high risk
? Assist in identifying gaps in care
? Collaborate with healthcare team to optimize patient adherence to medical plan of care
? Proactively outreach to members newly diagnosed with a targeted chronic condition
? Educate patient and family members about preventative screenings and identify barriers to care
? Interact with healthcare team
