This individual will be expected to monitor a specific group of patient?s care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients? needs are met.
A working knowledge of the clinical field is desired for this position. This is a Hybrid position, please note first week of training will take place in Scottsdale, AZ.
Care Manager Responsibilities: ? Manage a roster of patients who are enrolled in the Hybrid Patient Monitoring program (RPM). ? Act as a direct liaison between the patient and the office/provider. ? Scheduling and confirming appointments in office for the patient. ? Have a full understanding of each patient?s individual care plan and making sure they are following it. ? Record patient data daily and alert healthcare providers of abnormal reports.
Care Manager Qualifications: ? At least 1 year of clinical work experience as a Medical Assistant ? At least 1 year of EMR experience; eClinical

? High level of professionalism
? Must live in Arizona
? Excellent written and verbal communication skills
? Medical Assistant Course Completion Diploma or CMA designation required
