Clinical Case Manager

Responsibilities:
? Act as a liaison between patients, families, and healthcare providers, ensuring that all parties receive the assistance they need
? Coordinate the delivery of care services and facilitate the communication of relevant information between stakeholders
? Monitor patient progress, assess for barriers to care, and develop plans to address any identified issues
? Develop relationships with healthcare providers to ensure quality of care and positive outcomes
? Provide support to patients and families through education, counseling, and assistance with navigating the healthcare system
? Maintain accurate and up-to-date clinical records in accordance with applicable laws and regulations
? Monitor patient care plans and collaborate with healthcare providers to ensure patient needs are met
? Participate in quality improvement activities and initiatives to ensure best practices are followed
? Utilize managed care principles to ensure the most cost-effective care of patients
